Im a semi beginner when it comes to implants. Ive probably placed around 100…I placed two on my mom recently, #19, 30, and after about 3 weeks, #19 came out entirely. No pain, no issues, my mom woke up and rinsed her mouth and it just popped out. I know when i placed it, i wasnt subcrestal. It was an anyridge implant, im not at the office right now but i believe it was 4.5×10 (ish). Anyways, I know when i placed it was probably .5mm supracrestal (i know thats not ideal and not what i was going for), but it torqued to over 45 n/cm2. So far no issues with #30 at all (i know i was subcrestal with that one and also torqued over 45). I dont have a CT, and i know i need to get one, but i just bought a practice and those things are expensive as you know… any thoughts or ideas as to why this might have happened would be awesome. Can be daunting to ask about the screw ups sometimes…

Hmm, that’s tough to say. Implant failure to integrate depends on risk factors as well as the surgical technique and planning.

Because it could be several things, I am happy to list some common possibilities.

– was this done guided?

– what was the bone volume like? Did it have the minimum Dimensions required for that size implant? 1.5mm on buccal and lingual?

This commonly happens from trying to place into a ridge that was too narrow.

Another possibility is bone overheating. To help with this i use a drilling protocol that starts at 800 and work my way down to 300-400 rpm by the last drill. And I use a lot of irrigation.

A final possibility is impaired wound healing. This can happen with patients who have diabetes that is not well controlled or who are taking things like steroids for a long period of time.

Micro movement is another common one. Patients play with the implants if we put healing abutments on them sometimes.

And you know what? Sometimes we have no idea why. Shit happens.

As long as your mom doesn’t have any of the risk factors that rule out implants, and her health is okay, if should be safe to try again

Might have been an overheating issue…I stayed at pretty high rpm throughout until the end but then i went like 40rpm reverse no irrigation with the versuh bur…The ridge was solid width and the bone was good quality overall as well. She may have had some micromovement though, you never know lol!

Ill try again and see what happens. At least she wont give me a bad google review! If its gonna fail on someone im happy it was her and not someone else so i can still learn and not feel so embarrassed about it!
