what’s your protocol for immediate dentures if you’ve grafted some sites during extractions? In a recent case I performed gbr in one area and am hesitant to let the patient where the denture.

That’s a tough one, but I think I have some answers.

just relieve the crap out of the grafted area. You might even remove the flange there. Since you said its an immediate it makes it even harder to get retention. So I would advice also to do a soft reline and then going back to the soft reline in the GBR area and cutting that away. Its kind of crazy but you can actually remove some areas of a denture fairly aggressively and still have retention. You can just reline it back later on. Probably best to do a lab reline after the graft heals up.

I do it all right at delivery. It’s just so much easier knowing exactly the fit of the denture at that moment. Fine tuning it right then.

Cuz otherwise the patient is likely to go home and mess with it and have a terrible time with it. I like to try to make it easier for them.
