Hi guys, I have a patient with 4 existing implants upper left 12-15 and a bunch of crowns and bridge. He has to have all the remaining upper teeth removed and we're planning on an implant overdenture. He will get three more implants and I will remove one or two implant crowns. Then we will have 4-5 implants to support the denture. He wants to keep as many existing implant crowns as possible. What would be the ideal choice on which to keep and which to remove? Hey Doc, thanks for your feedback. I also thought about doing that at the beginning. I am new to overdenture, still in the middle of your course. Would you go for the most distal one, or would you use two of them, if we are going to remove all four crowns and abutments? Also, would there be a challenge if the angles of all implants are not consistent? Thanks a lot!

You are absolutely right. The most distal 2 seem like they would be angled better although it would probably be best to pick the most distal and most mesial just to get a better spread. The other 2 can just get healing abutments and the denture can rest on them.

It is possible to connect all of them to the denture but you are right, it can get tricky to seat everything.
