I placed #3, #6, #10. Others were placed before.

Initially, #5 was looking ok with X-rays, but I noticed buccal bone resorption at the time of placing #3, I did a bone graft and membrane hoping it would be ok.

Now, after 6 months when the patient came for the healing abutments stage, #5 implant screw is already showing through.

After X-ray it shows half of it already is exposed with no bone,

I started seeing the same problem with #7,

Do I need to remove those 2 implants before loading?

Do I need to leave them without including them with the fixed upper hybrid?

Or just remove #5? Can I leave it under the fixed without loading?

We all hope that our full arch implant cases last a long time and because of this I like to make sure to start from a very healthy and strong foundation. Implants with bone loss and thread exposure have a significantly higher incidence of issues down the road. They will almost always continue to lose more and more bone and can cause local destruction of the area.

At this point in the process, you might consider removing any implant that has more than 2 threads exposed.

In your case, it looks like 5 and 7 are candidates for removal.

I would not leave these be, and I would also not “bury” them because with this much bone loss I worry that they will cause issues to the surrounding area.
